  • Diabetes Prevention Programme for those with non-diabetes hyperglycaemia

  • Low Calorie Diet

  • ‘Referral to weight management service’

  • ‘Referral to National Health Service Diabetes Prevention Programme (procedure)’

  • ‘Referral to total diet replacement programme (procedure)’

Tier 2 Services Available

  • Better Health

  • Slimming World

A project was completed by the FCD which involved:

  • Scoping and data gathering – e.g. searching for eligible patients, requesting clinicians send any consenting patients via EMIS tasks to the FCD

  • Mapping of services we could refer into

  • Taking on patients who wouldn’t qualify for these services

  • Automating self-referrals through Accurx and attaching qualifying codes to those texts

Outcomes

  • The PCN reached max cap and therefore achieved full payment for the locally commissioned Weight Management Enhanced Service
