Overview
The weight management enhanced service offered £11.50 to the practice for every qualifying referral to a tier 2 weight management service in Reading
Qualifying referrals:
BMI 30kg/m (or 27.5 for patients with Black, Asian and other minority ethnic groups) – 40kg/m within the last 2 years
Diabetes Prevention Programme for those with non-diabetes hyperglycaemia
Low Calorie Diet
Qualifying Codes on EMIS
‘Referral to weight management service’
‘Referral to National Health Service Diabetes Prevention Programme (procedure)’
‘Referral to total diet replacement programme (procedure)’

A project was completed by the FCD which involved:
Scoping and data gathering – e.g. searching for eligible patients, requesting clinicians send any consenting patients via EMIS tasks to the FCD
Mapping of services we could refer into
Taking on patients who wouldn’t qualify for these services
Automating self-referrals through Accurx and attaching qualifying codes to those texts
Outcomes
The PCN reached max cap and therefore achieved full payment for the locally commissioned Weight Management Enhanced Service
